Florida school physical form (DH 3040): what parents need to know

You are reading the school supply list, and then you hit the line about a "school entry health exam" and a form with a number on it, DH 3040, and now it's July and you're calling offices hoping someone has an appointment before the first day. Here is the short version: the DH 3040 is Florida's school entry health exam form. Your child needs one if they are entering a Florida school for the first time. The exam has to happen within the 12 months before their first day; the completed form is due to the school within 30 days of entry. That is the general rule, but let's get to some of the details.

Who actually needs it

Children starting pre-K or kindergarten in Florida need it, and so does anyone transferring in from another state or country, at any grade. Some districts also recheck the requirement at certain grades, so if your school's registration list asks for it, the list wins. The DH 3040 is not an annual requirement. Once your child is enrolled in Florida schools, you won't be asked for a new one every August.

The two dates that matter

The exam must be done within the 12 months before your child's first day of school. That means a checkup from last summer may still count. Look at the date on the form before you book a new visit.

The completed form is due within 30 days of entry. Districts can give a temporary exemption of 30 school days for families who need it, but most schools ask for the form at registration, and starting the year with paperwork pending is a stress you don't need. Broward's first day is August 10 this year. Book in July and it's calm. Wait until late August and you're in the scramble with everyone else.

What the exam includes

A school physical is a regular checkup with a form at the end. Your child's height and weight, a vision and hearing screening, heart and lungs, and a review of their health history. If your child wears glasses, bring them. If you have records from a previous doctor, it's a good idea to bring those too. The visit is also a good moment to raise anything else that has been on your mind: a sleep problem, a picky eater or any other concern. The form only takes a few minutes, but the conversation is where the value is.

Don't confuse it with the DH 680

Florida schools ask for two separate documents. The DH 3040 covers the physical exam. The DH 680 is the Certification of Immunization, and it is a different form, issued through the state's immunization registry by a Florida provider. If your child's vaccine records come from another state or another country, a Florida provider can review them and transcribe them into a DH 680. Two forms, two boxes to check. If a school tells you something is missing, ask which of the two they mean.

A word on timing

Many families wait for August. By then the offices fill up, the urgent care lines grow, and the calm visit turns into a rushed one. Booking in July gets you a better appointment and leaves room to handle anything the exam turns up, like a vision referral, before school starts instead of during the first week.
